You will need to determine if your insurance covers physical therapy, how much is covered, what your co-pay is, and if you are required to have a referral. Please review this document prior to your first visit.

New Patient Questionnaire WITH Insurance

This is the informational form you will be asked to fill out on your first visit. Use this form if we are billing insurance. It provides us with our contact information, insurance information, and health issues. You will save time if you fill it out in advance and bring it with you! Also remember to bring your insurance card so we can photocopy it.

New Patient Questionnaire WITHOUT Insurance

This is the form you will be asked to fill out on your first visit. Use this form if we are NOT billing insurance, for example you are having a bike fit for performance (versus an injury).

HIPAA – Notice of Privacy Practices

We are required to ensure you are aware of our privacy policies, as mandated by Federal and State law. Please read and sign this document to prove you have read and understood the policy. This will be kept in your file. You will save time if you fill it out in advance and bring it with you to your first appointment!
